Easy Engineering and Simple Science Experiments for Little Kids
Fun experiments and challenges introduce kids to basic science and engineering concepts like acid and base reactions, color mixing, melting and freezing, buoyancy, density, viscosity, corrosion, magnetism, structural design, force and motion, aerodynamics, energy transfer, pulleys and levers, and many other topics. It's a hands-on blast! Early exposure to scientific thinking including hypothesis, observation, follow-up questions, and the importance of persistence and teamwork are core drivers of the experience.
Release and Waiver of Liability
Parties. The above-named Participant is a student enrolled in a Easy Engineering for Little Kids or Simple Science Experiments for Little Kids enrichment class (“Class”) provided by PolyKid Labs (“Organization”). The undersigned are parent(s) and/or legal guardian(s) of Participant (collectively, “Parent”).
Understandings. Parent and Participant understand that:
o Each science or engineering activity in Class requires the use of a handful of materials most commonly found in most households, such as paper, tape, scissors, rubber bands, tinfoil, toothpicks, paperclips, coins or tokens, popsicle sticks, water, string, plastic bags, straw, plastic cups, paper plates, duct tape, skewer stick, chopsticks, among other materials.
o In a minimal number of activities, Participants may need to do some cutting or make holes in materials. Such activities are recommended to be supported by the help of an adult, or to ensure proper supervision during such tasks.
o If used improperly, plastic bags may pose the risk of suffocation in young children, and small coins and tokens may pose the risk of choking.
o Scissors and toothpicks and skewer sticks have sharp ends and if used improperly may endanger Participant or other children or adults nearby, so must be handled with care.
o Rubber bands and other projectile materials may cause serious injury if used improperly, endangering Participant or other children or adults nearby.
o The use and access to minimal amounts of water may be needed in one or more activities, which if not supervised or controlled may pose a risk of flooding or drowning or property damage.
o Class is not staffed to monitor and supervise the activities of its participants at all times.
o There are inherent risks of serious personal injury involved in these activities as well as in general participation in Class programs, activities, and events.
Awareness and Assumption of Risk. Parent confirms Parent’s understandings as stated above. With such information and awareness, Parent knowingly authorizes Participant to participate in Class programs, activities, and events, and knowingly assumes and accepts, on behalf of Parent and Participant, the risks of all injury, illness, disease, death, property damage or loss, financial obligation, loss of privacy, and all other injuries and other consequences that may result, directly or indirectly, from Participant’s participation in Class programs, activities, and events, regardless of the cause.
Waiver and Release of Claims. Parent waives and releases Organization and its directors, officers, agents, employees, volunteers and affiliates (collectively, “Organization Parties”) from any and all liability, claims, costs, and damages of any kind which Parent, Participant, or Participant’s heirs, next of kin, or legal representatives may have or which may later accrue, caused by or arising directly or indirectly from (a) Participant’s participation in Class programs, activities, or events; (b) any first aid, transportation, or medical care received by Participant in connection with Participant’s participation; and (c) any travel by Participant in connection with Participant’s participation, (d) any property damage related to Participant’s participation in Class. This release and waiver includes, in each such case, all claims in respect of death, bodily or psychological injury, illness, loss of privacy, property damage, loss, financial obligation, or expenses, and in each case regardless of the cause or whether such claims arise from tort, contract or otherwise, and even if caused by negligence, whether passive or active. Parent and Participant agree not to sue any Organization Parties on the basis of these waived and released claims.
Consent to Photo and Video Release. The above-named Participant is a student in an Easy Engineering for Little Kids or Simple Science Experiments for Little Kids (“Class”) enrichment class, a class offered by PolyKid Labs (“Organization”). Parent(s) and/or legal guardians of Participant (collectively, “Parent”) consent to the recording of and use by Organization of Participant’s voice, image, and likeness, in any format, including photos, video, audio, print, or electronic (together, the “Materials”), without names or identifying information of Participant, and grant to the Organization the unrestricted, perpetual, and worldwide right and license to use the Materials for educational, archival, promotional, and other purposes as determined by Organization. Parent and Participant grant Organization all copyrights and other rights Participant or Parent may have in the Materials. Parent and Participant have all rights and licenses necessary to make these grants and agreements. Organization is not obligated to exercise any of the rights granted under this document. Organization’s use of the Materials may include, without limitation, publication, broadcasting, or posting in Organization’s website and social media platforms; dissemination through media outlets including television, radio, newspapers. and magazines; and publication in magazines, books, posters, and outdoor advertisements. Organization owns the physical Materials, as well as the content, any ideas or information they may generate, and the right to copy, edit, change, transfer, create derivative works of, or otherwise use the Materials as the Organization sees fit. Neither Parent nor Participant is entitled to receive any compensation for their use.
Waiver of Legal Claims. Parent waives and releases Organization and its directors, officers, employees, agents, and assigns from any legal claims, including, without limitation, any claims relating to copyright, rights of publicity or privacy, false light, or moral rights that Parent or Participant may have in the Materials or the Organization’s use and ownership of the Materials, and from any liability as a result of any distortion, blurring, alteration, optical illusion, or use in composite form, whether intentionally or otherwise, that may occur or be produced in the taking, processing, or reproduction of Materials or in their publication or distribution.
Acknowledgements and Additional Agreements. Parent acknowledges, confirms, and agrees to the above. Parent and Participant agree to opt in to communications about the Class or Agreement. Signature by one parent or legal guardian of this form, and use of electronic signatures on this form is sufficient for a fully executable agreement relating to Participant.
